As independent practitioners, records are separately maintained within each private practice. Visit the Providers link for direct contact information for each independent practitioner. Please allow 1-2 weeks for processing records requests.

Please click here for a Release of Information form and specify the name of the independent practitioner that holds the records. Parents must also complete this form for record keeping purposes.

Consider your past access to records if you have a Client Portal account with the provider as that is a quick and easy way to obtain existing records. Password reset requests can be made to your provider or by following the prompts on the log in. Dr. Forsyth and Dr. Schumann have Client Portal links on their provider pages for reference. If you do not have a Client Portal account, please contact the provider directly.
